神経膠腫とマクロファージの代謝、免疫相互作用

頭蓋内には神経細胞とグリア細胞があります。
このグリア細胞が癌化するのが神経膠腫(グリオーマ)です。
このグリオーマは小児の脳腫瘍のうち40%を占めると言わており、
成人に比べて、脳腫瘍の中で占める割合が高いと言われています。

この膠芽腫において予後不良を示すものの中には
骨髄性免疫細胞である単球由来のマクロファージとの相互作用によって
免疫機能が働きにくくなる寛容原生を示し、
それによって抗がん作用を示すT細胞の働きが抑えられます。
ー
この予後不良を示す膠芽腫は
イソクエン酸デヒドロゲナーゼ(IDH)と呼ばれる、
細胞のエネルギーの元となる代謝経路の
反応性を高める酸化還元酵素の役割が変異しています。

(敬称略)らドイツ、イギリス、アメリカの医療研究グループは
上述した膠芽腫と相互作用をするマクロファージが
変異した酸化還元酵素IDHが生成物である
R-2-hydroxyglutarate(R-2-HG)を通して
トリプトファンを分解する事を示し、
免疫寛容性に関わる受容体
アリル炭化水素受容体(Aryl hydrocarbon receptor)
を活性化させることを示しました。
このアリル炭化水素受容体は免疫細胞に発現されており、
免疫反応の環境、代謝の分子経路に関わります(2)。
また、トリプトファンの分解は
前述した免疫寛容に関わるとされています(3)。
従って、IDHの異常によりトリプトファンの分解や
AH受容体を活性化させる事が免疫の働きを弱めている可能性があります。
ー
ゆえに治療の戦略としては
トリプトファンの分解を防ぐことと
AH受容体の活性化を抑える事が挙げられます。
