B.1.351系統のノババックスワクチンの効果

2020年の11月くらいから南アフリカで広がりを見せている
新型コロナウィルスB.1.351系統は
逃避変異であるE484Kの他に
N501Y, K417Nなど世界で広がっている変異も入っています。
E484Kの変異は
従来の新型コロナウィルスで生まれた抗体に対する
結合親和性が低いことが示されているので
現在世界で接種されている、あるいは開発されている
ワクチンがどれくらい効力を示すのか？
世界の関心が集まっています。

ノババックス社(NVX-C0V2373)のナノ粒子ワクチンにおいて
V. Shinde, S. Bhikha氏ら南アフリカの医療研究グループは
2020年8月17～11月25日、16か所、南アフリカ
で行われた治験結果を示しています(1)。
人数は4387人で免疫不全のHIV陰性の人に絞っています。
ー
治験で感染した患者41人のうち38人は
B.1.351系統の新型コロナウィルスであったため
治験のワクチンの効果の結果は
ほぼこの逃避変異を含む系統のデータを示している
と考えられます。
ー
ワクチンの効果は50～60%程度です。
評価の基準は2回接種後7日です。
従って、1回目接種から28日経過しており、
抗体が十分に高まっている時期に調べられています。
ブレークスルー感染した患者の
症状の程度は記載されていません。
ワクチン接種では重症化の予防も期待されるため
それがどの程度効果があるか
というのは一つ疫学、社会、医療的に重要です。

Ad26.COV2.S  vaccine  (Johnson  & Johnson/Janssen)
これについては6576人(南アフリカ)のデータで
1回接種後28日でB.1.351系統に対して64%の
感染予防効果がありました。

ChAdOx1 nCoV-19 vaccine  (AstraZeneca)  
これについては2026人の参加者に対して
南アフリカでB.1.351系統に対する
感染予防効果は小さい(10%程度)でしたが、
ブレークスルー感染した人の
症状は軽症から中程度で抑えられています(2)。
